Schedule / Job TypeContract | Days | 5×8-Hour Shifts (8:00-16:30) | Onsite
Key Responsibilities- Provide case management services within an acute care hospital setting
- Coordinate patient care planning, discharge planning, and utilization review
- Float between like units based on facility need
- Document and chart patient information using Epic EMR
- Participate in every-other-weekend (EOW) scheduling rotation
- Commit to working 3 of 5 designated holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
Required Qualifications- Active RN license required; compact license must be in hand at time of submission
- Minimum of 2 years of case management experience; at least 1 year required - first-time travelers considered with clinical approval
- Acute care experience required
- Must submit all requested time off (RTO) at the time of submittal; post-offer RTO may not be considered
- 2 professional references covering 1 year of experience within the last 3 years required prior to start
- Local candidates are welcome; candidates living within 50 miles of the facility may be accepted at a different rate
Preferred Experience / Skills- Prior experience with Epic EMR charting system
- Experience floating between units or sister facilities
- Strong communication and care coordination skills in a fast-paced acute care environment
