Schedule / Job TypeContract | Days | 5×8-Hour Shifts (8:00-16:30) | Every Other Weekend Required
Key Responsibilities- Provide case management services within a hospital acute care setting
- Coordinate patient care plans, discharge planning, and utilization review
- Document and chart patient information using Epic EMR
- Float between like units based on facility need
- Collaborate with interdisciplinary teams to support patient outcomes
- Commit to working 3 of 5 designated holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day
Required Qualifications- Active RN license required; compact license must be in hand at time of submission
- Minimum of 2 years of case management experience; at least 1 year required per facility compliance
- Acute care experience required
- Proficiency with Epic EMR
- Must submit all requested time off (RTO) at the time of submittal - post-submittal RTO may not be considered
- Local candidates are welcome; must live within 50 miles of the facility
Preferred Experience / Skills- Prior experience in hospital-based case management or care coordination
- Familiarity with utilization management and discharge planning processes
- Experience working in a travel or contract nursing capacity
