Schedule / Job TypeContract | Days | 5×8-Hour Shifts (8:00-16:30) | Every Other Weekend Required
Key Responsibilities- Provide case management services within a hospital acute care setting
- Coordinate patient care plans, discharge planning, and utilization review
- Collaborate with interdisciplinary teams to ensure continuity and quality of care
- Document all case management activities using Epic EMR
- Float between like units based on facility need
- Commit to working 3 of 5 designated holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
Required Qualifications- Active RN license required; Compact license must be in hand at time of submission
- Minimum of 1 year of RN experience required; 2 years preferred
- Acute care experience required
- Proficiency with Epic EMR
- 2 professional references covering at least 1 year of experience within the last 3 years required prior to start

Preferred Experience / Skills- 2 or more years of case management experience in a hospital setting
- Experience with utilization management and discharge planning
- Familiarity with multi-facility or system-level healthcare environments
