Description:
Urgent need for Registered Nurses in Home Health.
Candidates must have a comprehensive understanding of health status evaluations and the ability to make determinations based on assessment criteria for enrollment.
Nurses should be proficient in technology and have knowledge of nursing practices, state laws, and agency regulations.
Excellent organizational, interpersonal, written, and verbal communication skills are essential.
Candidates must effectively communicate with diverse consumers and show sensitivity to their concerns.
The ability to work independently in a fast-paced and deadline-oriented environment is necessary.
Responsibilities include performing initial in-home pre-evaluations of patients, assessing health status, strengths, care needs, and preferences.
Nurses will review medical documentation and enter evaluation data into electronic forms.
Documenting any concerns and issues during evaluations is required.
Emphasis will be placed on continuity of care, reducing fragmentation and gaps in treatment plans.
Candidates will discuss health care options, supports needed, and service vendor options with consumers.
They will communicate with consumers for enrollment in managed care health plans.
Enhancing communication with interdisciplinary team members is crucial for improved care coordination.
Collecting quality review data and required documentation for outcome measurements is part of the role.
Identifying opportunities for health promotion and illness prevention is important.
Maintaining comprehensive knowledge of community resources is necessary.
Meeting established performance standards is expected.
Additional duties may be assigned by management.
