Candidates must possess a strong understanding of UAS criteria for enrollment. Ability to work with technology in the workplace is essential. Knowledge of the principles and practices of professional nursing is required. Familiarity with state and agency laws and regulations is expected. Excellent organizational, interpersonal, written, and verbal communication skills are necessary. Candidates should be able to communicate effectively with a diverse population. Sensitivity to the concerns of others is vital. The ability to thrive in a fast-paced, deadline-oriented work environment is required. Candidates must be capable of managing multiple complex tasks simultaneously. A strong ability to work independently is essential. Job Responsibilities: Perform initial in-home evaluations of patients, assessing health status, strengths, care needs, and preferences. Guide the development of individualized long-term care service plans. Review consumer medical documentation and health referral forms as relevant. Enter evaluation data into electronic forms and transmit as required. Document any concerns or issues that surface during evaluations. Emphasize continuity of care to reduce fragmentation and gaps in treatment. Discuss available healthcare options, supports needed, and service vendor options with consumers. Initiate communication for enrollment into managed care health plans. Enhance collaborative relationships with interdisciplinary care team members to improve service delivery. Collect quality review data and documentation to support outcome measurements. Identify opportunities for health promotion and illness prevention. Maintain knowledge of community resources and services available to the target population. Meet all performance standards established for this position. Perform other duties as may be assigned by management.
