Job Title: Registered Nurse Profession: RN Specialty: Home Health Duration: 260 days Shift: 5x5 days Hours per Shift: Not specified Experience: 2 years of recent experience in a home health setting within the last 15 years License: Current RN Licensure in Client Certifications: BLS required before start date Must-Have: Bilingual Spanish-English proficiency strongly preferred Description: URGENT NEED Candidates will conduct hybrid telehealth and in-field assessments. Candidates cannot do UAS assessment assignments elsewhere. Candidates must sign a confidentiality form before starting. Ability to review health status questions and make evaluations based on criteria for enrollment. Ability to work with technology in the workplace. Knowledge of the theory, principles, and practices of general professional nursing. Knowledge of state and agency laws and regulations governing professional nursing practices. Excellent organizational, interpersonal, written, and verbal communication skills. Ability to communicate and work effectively with multi-generational consumers. Ability to demonstrate sensitivity to the concerns of others. Ability to perform comfortably in a fast-paced, deadline-oriented work environment. Ability to successfully execute many complex tasks simultaneously. Strong ability to work independently. JOB RESPONSIBILITIES Perform initial in-home pre-evaluation of patients, including health status, strengths, care needs, and preferences. Guide the development of individualized long-term care service plans. Review consumer medical documentation and relevant health referral forms. Enter evaluation data into an electronic evaluation form and transmit as required. Document any concerns or conflicts in information during the evaluation process. Emphasize continuity of care, reducing fragmentation, duplication, and gaps in treatment plans. Discuss health care options, needed supports, service vendor options, and waiver options with consumers. Initiate communication for enrollment into a managed care health plan. Enhance communication and collaborative relationships with the interdisciplinary care team. Collect quality review data and required documentation to support outcome measurements. Record case notes into the Customer Relationship Management system. Identify opportunities for health promotion and illness prevention. Maintain a comprehensive working knowledge of community resources and network services for the target population. Meet all standards established for this position. Perform other duties as assigned by the Regional Nurse Manager or project management.
