We are passionately committed to assisting patients and families in achieving optimal health, independence, and comfort through high quality, cost-effective home and community health-related services.POSITION:PT Home HealthLOCATION/S: Missoula, Missoula County | Hamilton and Stevensville, Ravalli County | Ronan and Polson, Lake CountySTATUS: Full-Time | 40 hours/week possible callSCHEDULE: Typically Monday through Friday, 8am-5pm.This position participates in rotating on-call, weekend, and holiday coverage.Job Physical Therapist is responsible for evaluation and rehabilitation intervention in the home setting, including management and supervision of patient care and patient and family education.Essential Duties and Responsibilities:1. Evaluates and establishes plan of care to meet patients needs in the home environment. Coordinates care with other disciplines and seeks orders for appropriate additional services.2. Provides direct patient care based on the established care plan and physician orders.3. Re-evaluates patient status and progress. Modifies treatment as appropriate, including frequency and duration of care.4. Communicates appropriately with physicians and other staff members.5. Documents all skilled services provided to patients.6. Communicates with scheduler on each day worked.7. Maximizes consistency of care when preparing schedules.8. Oversee services provided by Physical Therapist Assistant (if any).9. Performs other duties as assigned by supervisor.Minimum include BS degree or better from an accredited Physical Therapy program with current license to practice in the State ofMontana, valid driver s license, and appropriate auto insurance.
