• Provide skilled nursing care to patients in their homes, ensuring continuity and quality of care.
• Assess, plan, and implement individualized care plans for each patient.
• Educate patients and families on treatment plans and preventive health measures.
• Coordinate with interdisciplinary teams to ensure optimal outcomes and patient safety.
6 months–1 year of recent Home Health experience required
BCLS certification required
Must obtain MOAB Certification (4-hour de-escalation course provided first week)
COVID vaccination required (medical/religious exemptions accepted)
Must pass CYFD fingerprinting upon arrival (home visits may involve children)
