- Provide skilled nursing care and case management to patients in their homes.
- Assess, plan, implement, and evaluate individualized patient care plans.
- Monitor patient conditions and respond to changes in health status.
- Coordinate care with physicians, therapists, home health aides, and other healthcare professionals.
- Educate patients and families on disease management, medications, and treatment plans.
- Supervise Home Health/Hospice Aides and CNAs to ensure quality patient care.
- Maintain accurate documentation in compliance with OASIS and agency standards.
- Participate in on-call rotation and provide home visits as needed.
