Job Summary

We are seeking an experienced Home Health Registered Nurse (RN) for a long-term assignment serving the Manistee area.

The ideal candidate will have experience providing care in Medicare Certified Home Health Care settings and be comfortable working independently in patients' homes. Experience with Homecare Homebase (HCHB) software is preferred.

Responsibilities

• Provide skilled nursing care to patients in their homes.

• Perform comprehensive patient assessments and develop individualized care plans.

• Provide treatments and interventions according to physician orders and established care plans.

• Monitor patient conditions and communicate changes to physicians and the healthcare team.

• Educate patients and families regarding medications, treatments, disease management, and home safety.

• Complete accurate and timely clinical documentation.

• Coordinate care with other members of the home health team.

• Provide care in accordance with Medicare-certified home health requirements and agency policies.

• Maintain professional standards while working independently in the field.

 

Schedule

• Monday–Friday, 8:00 AM–5:00 PM.

• 40 hours per week.

• Every 4th weekend.

• During a week that includes a weekend workday, the schedule will be reduced by one weekday before and/or after the weekend assignment.

• 1 winter holiday and 1 summer holiday required.

• No on-call requirement.

• No guaranteed hours.

• Overtime after 40 hours.
