HOME HEALTH VISIT RN – PROHEALTH CARE

Location: Waukesha, WI
Shift: 8:00 AM–6:30 PM | Sat–Tue | 10 hrs/day
Contract: 13 Weeks | 40 hrs/week
Start: 8/10
Patient Ratio: 1:1 | Adults 18+
Travel: 7–7.5 visits/day | 50–100 miles/day
On-Call: None

Requirements:

• 1–2+ years current Home Health & Hospice experience – REQUIRED
• Home Health/Hospice admission, death visits & symptom management
• Recent OASIS experience – REQUIRED
• Epic experience within 12 months – REQUIRED
• Active WI RN License
• AHA BLS
• Valid Driver’s License
• Associate Degree/Certificate

Other: Home care experience required; flexible/autonomous; strong assessment & documentation skills; comfortable driving 50–100 miles/day and navigating stairs/uneven terrain. Interview: Manager Phone Interview + MSP Interview Holiday: As needed COVID vaccine required unless eligible exemption.
