Registered Nurse – Home Health | Redding, CA | 13 Weeks Contract

Position: Registered Nurse – Home Health
Location: Redding
Contract Length: 13 Weeks
Shift: Day Shift
Schedule: 5×8
Experience Required: 2 Years
Weekly Pay:$2870 to $3070
Responsibilities
Perform comprehensive in-home patient assessments and develop individualized care plans
Provide skilled nursing care including wound care, medication administration, IV therapy, catheter care, and disease management
Monitor patient progress and modify care plans in collaboration with physicians and interdisciplinary healthcare teams
Educate patients and caregivers regarding medications, treatments, chronic disease management, and home safety
Coordinate care with physical therapists, occupational therapists, speech therapists, social workers, and case managers
Complete accurate and timely clinical documentation in the electronic medical record (EMR)
Assess home environments and recommend interventions to improve patient safety and independence
Communicate changes in patient condition promptly to physicians and supervising clinicians
Participate in discharge planning and ongoing patient care coordination
Maintain compliance with home health regulations, infection prevention standards, and agency policies
Deliver compassionate, patient-focused care while promoting independence and improving quality of life
Requirements
Active California Registered Nurse (RN) License
Minimum 2 years of recent Home Health nursing experience
Current BLS Certification required
Valid driver's license, reliable transportation, and proof of auto insurance for patient home visits
Strong assessment, wound care, medication management, and patient education skills
Experience with OASIS documentation preferred
Experience using electronic medical record (EMR) systems
Excellent communication, time management, and organizational skills
Ability to work independently while managing a daily patient schedule
