We are currently seeking an experienced PACE Admissions Registered Nurse (RN) for a 3-month contract assignment in San Diego, CA. The ideal candidate will have experience in home health or community-based nursing and be comfortable conducting in-home assessments, completing Level of Care (LOC) evaluations, and coordinating care for the frail elderly population.
Position Details:

Contract Length: 3 Months (With high possibility of extension)
Schedule: Monday – Friday | 8:00 AM – 5:00 PM
Guaranteed Hours: Full-Time (40 Hours Per Week)
Weekend Requirement: None
On-Call Requirement: None
Travel: Required for Home Site Visits
Requirements:
- Active California RN License required
- Min 1 year experience as an RN
- Home Health or Community Health experience preferred
- Experience conducting comprehensive nursing assessments
- Ability to complete Level of Care (LOC) evaluations
- Valid driver's license and reliable transportation
- Experience coordinating home care services and durable medical equipment (DME) preferred
- Strong documentation and care coordination skills
Skills & Qualifications:
- Experience performing in-home patient assessments
- Strong clinical assessment and critical thinking skills
- Knowledge of care planning and case management
- Ability to work independently in the community setting
- Excellent communication and patient/family education skills
- Strong organizational and time management abilities
- Experience working with the geriatric or frail elderly population preferred
