Description
- Provide skilled nursing care to patients in their homes based on individualized care plans.
- Perform comprehensive patient assessments and monitor changes in patient condition.
- Complete accurate and timely OASIS assessments and documentation.
- Provide patient and family education regarding medications, treatment plans, disease management, and self-care.
- Coordinate care with physicians, caregivers, and other members of the interdisciplinary healthcare team.
- Maintain accurate clinical documentation and comply with home health regulations and agency standards.
- Manage patient visits efficiently while ensuring high-quality, compassionate care.
