Provides clinical care through the direct application of the nursing process; performs and documents resident assessments and progress notes, evaluates changes in care needs, completes Individualized Service Plans (ISP)/Individualized Care Plan (ICP), provides or delegates hands-on clinical care as indicated by the plan of care, and evaluates resident outcomes. Medication Management: Provides strategic leadership for resident care in the community by managing, directing, and monitoring the medication care managers, as applicable by State/Provincial law, to promote the health and wellness of the resident population.
