Registered Nurse - Hospice & Home Health
Job Summary
Join our compassionate team in Missoula, Montana, as a Registered Nurse specializing in Hospice and Home Health. This role involves providing comprehensive skilled nursing care and expert case management to patients within their residences and at our dedicated in-patient Hospice Center. You will play a vital role in supporting patients and their families through end-of-life care, focusing on comfort, dignity, and quality of life.
Leveraging the nursing process, you will conduct thorough assessments, develop individualized care plans, and implement interventions within an interdisciplinary team framework. Responsibilities include admitting new patients, coordinating integrated services to ensure seamless continuity of care, and facilitating optimal clinical and cost outcomes. You will also be instrumental in negotiating and procuring necessary resources for patients and their families.
Key Responsibilities
- Provide skilled nursing care and coordinate services for hospice patients and their families in residential and in-patient settings, based on physician authorization and systematic evaluation.
- Conduct comprehensive physical and psychosocial assessments, observe changes in condition, and plan interventions related to treatments or medications.
- Oversee and evaluate care provided by Home Health/Hospice Aides and Certified Nursing Assistants (CNAs), ensuring compliance with established care plans.
- Ensure continuity of care through effective communication, information exchange, and case conferences with other disciplines, community health, and social agencies.
- Deliver quality care and appropriate symptom management, promoting positive clinical outcomes through patient/family teaching, counseling, and referrals.
- Maintain accurate, current, and complete patient records in accordance with agency standards and OASIS requirements, adhering to current reimbursement regulations.
- Participate in team scheduling coordination and an on-call rotation, assessing patient needs via telephone and conducting home visits as required.
- Engage in quality improvement activities and attend assigned in-service training sessions.
Required Qualifications
- Current Registered Nurse (RN) license in the State of Montana.
- Valid Driver's License.
- Appropriate automobile insurance.
- Reliable transportation.
Preferred Qualifications
- Bachelor of Science in Nursing (BSN).
- Two (2) years of recent experience in acute care, home care, or hospice settings.
Skills & Competencies
- Clinical Skills: Venipuncture, catheterization, wound care, parenteral therapy.
- Core Competencies: Comprehensive patient assessment, care planning, intervention, evaluation, symptom management, patient/family education, interdisciplinary collaboration, effective communication, accurate documentation, case management, critical thinking, adaptability.
