Job Requirements
  • Education Requirements: Valid RN license in the state of Wisconsin.
  • Certifications Requirements: BLS certification (American Heart Association or equivalent), EPIC certification (or willing to obtain).

Job Responsibilities
  • Provide patient assessments and symptom management for end-of-life care, including pain and shortness of breath.
  • Collaborate with physicians and healthcare teams to develop and implement individualized hospice care plans.
  • Educate patients and families on hospice benefits, medication management, and end-of-life philosophies.
  • Assess referral patients, determine appropriate hospice level of care, and facilitate admissions within hospital or home settings.
  • Travel to various hospitals and facilities within the designated region to provide consults, assessments, and hospice services as needed.
