Description:
Provides coordination and direction for home nursing care according to physician's orders.
Care is based on agency policies and the competent application of the nursing process.
Responsible to the Clinical Manager.
Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses.
Home visits and periodic office meetings required.
Must have reliable transportation, a current driver's license, and agency-required liability insurance.
Average number of patients per day is 5-8.
Contractors will be assigned between 5-10 patients per day.
Types of cases include Start of Care, Wound Care, and Follow-up Visits.
First-timers are not accepted; recent specialty experience is required.
Weekend requirements vary per location and should be discussed during the interview.
No on-call requirement for this position.
Must be willing to travel up to 75 miles from the home clinic.
Mileage is reimbursed at the IRS standard rate.
Contractors will travel from patient home to patient home, excluding the commute to and from the first and last patient.
Local candidates are accepted at the same rate.
Pending licenses are not accepted; candidates must have their license in hand.
Candidates should be proficient in HomeCare HomeBase and Oasis, having used these systems within the last six months.
All documentation must be completed within 24 hours of patient appointments.
Delayed or incomplete documentation affecting billing and follow-up care may result in cancellation of assignments.
There are no guaranteed hours with this contract; contractors must meet weekly productivity points of 30, which equates to 40 hours per week.
Alternatives may be available to ensure financial needs are met through floating to another clinic if patient volume is low.
