Description:
Provides, coordinates, and directs the provision of home nursing care according to physician's orders.
Care is based on agency policies and procedures through the competent application of the nursing process.
Responsible to the Clinical Manager.
Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
Conducts home visits and attends periodic office meetings.
Must have reliable transportation, a current driver's license, and agency-required liability insurance.
Average number of patients per day is between 5-8.
The contractor will be assigned between 5-10 patients per day.
Each type of visit and activity has a different point value associated with it.
Types of cases include Start of Care, Wound Care, and Follow-up Visits.
Expectations include acceptance of every patient scheduled, regardless of distance.
All documentation must be completed within 24 hours of the appointment.
No guaranteed hours are available with this contract.
The clinician must hit weekly productivity points to support their financial needs.
Willingness to travel up to 75 miles from the designated care center is required.
Local candidates are accepted at the same rate.
Proficiency in relevant electronic medical record systems is required.
Candidates must be open to on-call rotations.
The position does not have a weekend rotation requirement.
