Travel Nurse RN - Hospice - $2,327 per week in Newport News, VA

Job Title: Hospice Nursing Case Manager Profession: Registered Nurse Specialty: Hospice Duration: 13 weeks Shift: Day 5x8-Hour Hours per Shift: 8 Experience: Must have experience with home hospice case management License: Active nursing license required Certifications: BLS required, EPIC experience preferred Must-Have: Must have experience with home hospice case management, no exceptions Experience with managing a patient census of approximately 35-45 patients Description: RN case manager needed for hospice program. The average daily census is 150 patients. Visits are made to patients in both private homes and facilities. The RN is scheduled with five points per day. Revisits count as one point and admits count as 2.75 points. Visit types and their corresponding point values include the following: - Death at home: 0.5 points - Discharge without visit: 0.5 points - Discipline discharge: 1 point - Follow-up: 2 points - Initial evaluation: 2 points - Lab draw: 0.5 points - Oasis discharge: 1.5 points - Preceptor day: 6 points - Recertification: 2 points - Resumption of care: 2 points - Start of care: 2.75 points Floating may be requested based on the needs of the facility. All units require specialty experience within the last year. Candidates must provide proof of certifications and licenses upon application. Prior employees must be gone for at least six months before being hired for travel roles. RTO must be kept to a minimum. Any requests for time off must be included in the initial submission.
