Join a dedicated hospice team in Missoula, Montana, as a Registered Nurse Case Manager. This role involves providing comprehensive skilled nursing and compassionate case management to patients and their families in their place of residence. You will play a crucial role in coordinating care, ensuring continuity, and facilitating optimal clinical and quality-of-life outcomes through an interdisciplinary approach.
The position focuses on patient assessment, planning, implementation, and evaluation of care, with a strong emphasis on patient and family education. While primarily a Case Manager role, the position may initially involve duties as a Visiting RN, with the same core clinical responsibilities.
Key Responsibilities
Provide skilled nursing care and coordinate services for hospice patients in their residential settings, based on physician authorization and an interdisciplinary team approach.
Conduct thorough assessments of patients' physical and psychosocial statuses, including changes in condition, and develop individualized care plans.
Implement and evaluate interventions, monitor patient responses to treatments and medications, and adjust care plans as necessary.
Oversee and evaluate the care provided by Home Health Aides and Certified Nursing Assistants (CNAs) to ensure adherence to the plan of care.
Ensure seamless continuity of care through effective communication, information exchange, and case conferences with other disciplines and community resources.
Promote optimal health and quality of life through patient and family teaching, counseling, appropriate interventions, and referrals, focusing on symptom management.
Maintain accurate, current, and complete patient records in accordance with agency standards, OASIS requirements, and current reimbursement regulations.
Participate in team scheduling coordination and an on-call rotation, which includes telephone assessments and making home visits as required to meet patient care needs.
Engage in quality improvement activities and attend mandatory in-service training sessions.
Required Qualifications
Valid driver’s license, appropriate auto insurance, and reliable transportation.
Preferred Qualifications
Bachelor of Science in Nursing (BSN).
Two (2) years of recent experience in acute care, home care, or hospice settings.
Proficiency with Homecare Homebase (HCHB) electronic medical record system.
Skills & Competencies
Clinical Skills: Venipuncture, catheterization, wound care, parenteral therapy, comprehensive patient assessment, pain and symptom management, end-of-life care planning.
Soft Skills: Excellent communication and interpersonal skills, critical thinking, problem-solving, strong organizational and time management abilities, ability to work autonomously and collaboratively within an interdisciplinary team, patient and family education, empathy, and cultural sensitivity.
