Job Opportunity – Registered Nurse Case Manager – Montana

Start Date: September 16, 2026

Location: Missoula, Montana

Compensation: Details provided to qualified applicants

Schedule: Monday through Friday, 8:00 AM – 5:00 PM, with on-call and weekend rotation. 8-week contract assignment.

Highlights:

• Provide skilled nursing care and case management to patients in their homes through an interdisciplinary team approach
• Coordinate patient care services, ensuring continuity and quality clinical outcomes across the care continuum
• Oversee care delivery by Home Health Aides and CNAs, ensuring compliance with individualized care plans
• Utilize OASIS documentation standards and maintain accurate patient records in accordance with regulatory requirements
• May begin as a Visiting RN with progression to Case Manager role; clinical responsibilities apply to both designations
• Participate in quality improvement initiatives and ongoing professional development opportunities

Requirements:

• Current Registered Nurse license in the State of Montana (BSN preferred)
• Valid driver's license with appropriate auto insurance and reliable transportation
• Two years of recent acute care, home care, or hospice experience preferred
• Clinical competency in venipuncture, catheterization, wound care, and/or parenteral therapy preferred
• HCHB (Home Care Homebase) experience strongly preferred
• Knowledge of current reimbursement rules and regulations related to home health services
