Job Requirements

• Current RN license in the state of Montana
• 1+ year of recent acute care, home care, or hospice nursing experience
• BSN preferred
• Valid driver's license
• Appropriate auto insurance
• Reliable transportation
• Experience with venipuncture, catheterization, wound care, and/or parenteral therapy preferred
• Strong communication, documentation, and care coordination skills

Responsibilities

• Provide skilled nursing care and case management for patients in their residence or inpatient hospice setting
• Admit patients to services and coordinate care plans with the interdisciplinary healthcare team
• Assess, observe, and evaluate patient physical and psychosocial status, including changes in condition
• Plan and implement nursing interventions based on physician-authorized care needs
• Oversee care delivered by Home Health and Hospice Aides and CNAs
• Promote continuity of care through effective communication with other disciplines and community resources
• Provide teaching, counseling, symptom management, and appropriate referrals to support quality of life
• Maintain accurate and timely patient records in accordance with agency standards and OASIS requirements
• Participate in team scheduling, on-call rotation, quality improvement activities, and in-service training
