Job Title: Registered Nurse - Rehab

Description:
Delivers coordinated nursing care for patients according to established standards of care and the nursing process.
Supervises and directs the activities of various levels of assigned nursing staff.
Coordinates care with other disciplines while utilizing critical thinking and professional discretion.
Employs independent judgment in patient care delivery.
Collects relevant data pertinent to patients' health or situation.
Analyzes assessment data to determine diagnosis and care issues.
Develops plans that prescribe interventions to attain desired outcomes.
Implements plans, coordinates care delivery, and utilizes strategies to promote health and safety.
Evaluates progress toward attaining outcomes.
Identifies suitable outcomes for patients' situations.
Collaborates with patients, families, and healthcare providers to ensure safe and humane care.
Provides learning opportunities for patients, family members, and team members.
Conveys health information to patients, families, and the treatment team effectively.
Participates in discharge planning to ensure continuity of care.
Delegates responsibilities appropriately and coordinates duties of healthcare team members.
Performs other job-related duties as assigned.
