Description:
Conduct health assessment interviews focused on wound, ostomy, and continence related care.
Perform focused physical assessments and develop plans of care based on individual client needs.
Recommend medications and treatments as appropriate.
Develop, revise, and evaluate standards of care.
Assist in the selection of supplies, technology, and equipment.
Assess and document patient responses to wound care, ostomy, continence, fistula, and complex wounds treatment.
Evaluate the quality and delivery of care provided by clinical staff.
Collaborate with the healthcare team and make referrals to other disciplines as indicated.
Teach, counsel, and assist individuals and families in assuming responsibility for prevention of complications.
Develop, provide, and evaluate staff education programs, including new employee orientations and continuing education.
Conduct comprehensive assessments of patients at risk for developing pressure injuries and initiate preventative measures.
Communicate challenges identified, including ethical issues and conflict resolution.
Collaborate to initiate and recommend best practices based on assessments.
Participate in unit and department quality improvement activities.
Provide culturally competent education consistent with diverse belief systems.
Maintain the security and confidentiality of patient information.
Achieve and maintain professional competency by engaging in relevant activities.
Identify community resources for clients and assist in utilizing such resources.
Manage assets related to rented equipment, such as specialty beds and negative pressure devices.
