Key Responsibilities- Evaluate patients and develop individualized occupational therapy treatment plans within a skilled nursing facility setting
- Provide direct occupational therapy services to residents to restore, maintain, or improve functional independence
- Document patient progress, treatment plans, and outcomes in accordance with facility and regulatory standards
- Collaborate with interdisciplinary care teams to coordinate patient care and discharge planning
- Educate patients and family members on therapeutic techniques and home exercise programs
Required Qualifications- Current and valid Occupational Therapist (OT) licensure in the state of California
- Graduate of an accredited Occupational Therapy program
- Experience working in a skilled nursing or post-acute care environment
Preferred Experience / Skills- Experience in a small or community-based SNF setting
- Strong documentation and communication skills
- Ability to work independently while contributing to a collaborative care team
