Description:
This position is responsible for the evaluation, planning, administration, and supervision of Occupational Therapy treatment in accordance with physician referrals, regulatory standards, and established departmental policies and procedures.
The role requires demonstrating clinical competence and knowledge necessary to provide age-appropriate treatment to patients served.
Evaluates patients' functional abilities and limitations.
Determines the need for intervention.
Establishes a plan of care and treatment goals, completing comprehensive evaluations within established time frames.
Identifies and prioritizes key limiting factors.
Collaborates with patients and their families/caregivers when setting goals.
Initiates discharge planning and recommends modifications to referring orders as needed.
Implements individualized treatment plans considering patients' physical, social, spiritual, cultural, educational, and age-specific needs.
Provides skilled treatment in a safe manner while assessing ongoing treatment effectiveness.
Modifies plans of care as indicated and involves patients and families/caregivers in the treatment process.
Provides ongoing education to the interdisciplinary team, including physicians, nurses, patients, and families/caregivers.
Documents according to standards set by regulatory agencies and the department.
Ensures legibility and timeliness of documentation.
Records innovative and individualized treatment techniques and reflects a comprehensive assessment of patients' progress.
Reassesses treatment goals in accordance with departmental policies.
Communicates pertinent patient information to ensure optimal continuity of care.
Effectively relays evaluation and treatment results to relevant personnel.
Takes responsibility for contacting physicians for problem-solving to achieve optimal patient outcomes.
Exhibits adaptability in assignments related to caseload, scheduling, and staffing shifts.
Initiates assistance in other patient care areas and accepts responsibility for all patient care needs.
Demonstrates appropriate direction and delegation of personnel to ensure safe delivery of quality patient care.
Participates in the training and orientation of new personnel and assures their competency prior to task delegation.
Engages in departmental operational activities and participates in process improvements.
Meets productivity standards as established by the department.
Interacts with various levels of staff, patients, families, and external contacts concerning the health care of patients.
Communicates effectively with other health care institutions and community providers to ensure comprehensive patient care.
