LOCATION: Baylor University Medical Center, Dallas, Texas

SPECIALTY/DEPARTMENT: Advanced Treatment and Transplant Clinic, the primary care is for heart failure patients.

SHIFT/SCHEDULE: Monday-Friday, FT Days, 8- or 9-hour day some flexibility. There is an on-call one week every 6 weeks.

This position is 90% travel to locations throughout Texas to see patients, overnight stay at times. Exact details to be discussed in a formal interview.

Job Summary

As a Transplant Coordinator RN, utilize a nursing model for quality patient care. Assess patient needs and create personalized care plans. Watch how patients respond to personalized care. Ensure patients have smooth transitions through healthcare. Prevent complications or delays by understanding needs.

Essential Functions of the Role

  • Assessing transplant eligibility for patients involves interviewing them and their families.
  • You will coordinate tests for transplant candidates, identify issues in results, and arrange necessary care. You''ll work closely with patients, physicians, and other healthcare providers during this time.
  • Manage recipient''s post-transplant care.
  • Arrange admissions, lab tests, clinic visits.
  • Coordinate with referring, primary care. Understand test results, identify irregularities.
  • Set up needed care as directed.
  • Teaching candidates, families, caregivers about evaluation, organ allocation, waiting, post-operative, immunosuppression effects, complications, and psychological impact of transplant.
  • Using insurance knowledge to help with post-transplant prescriptions. Assisting patients with coverage issues.
  • Maintaining long-term prescriptions.
  • Complying with various regulatory policies and maintaining records on the transplant waitlist through different online programs.
  • Coordinating recipient transplants include using the donor website and communicating with potential recipients. Assessing their health status and notifying hospital departments are important tasks. Updates are also sent to the team.

  • Knowing rules, laws, regulations, standards, guidelines, certifying bodies, accrediting bodies relevant to our department is important.
  • Knowledge of medical terms, health practices, disease control, medication specs, and side effects is essential for healthcare professionals.
  • Respect for, and diligent observance of, professional nursing ethics and patient privacy rights.
  • Excellent communication skills, both spoken and written, for clear transmission of ideas.
  • Basic computer skills: Microsoft Office, information security, scheduling systems, payroll systems, electronic medical records, and email use.

REQUIRED QUALIFICATIONS

  • Associate''s degree level.
  • 2+ years of experience working with heart failure patients, or ICU in an inpatient setting
  • Willingness to travel (overnight stay may be required at times)
  • An (RN) Registered Nurse License no stipulations or restrictions.
  • Basic Life Support (BLS) and ALS skills required or obtain within (30) thirty days if not already held.
