Contract | Full-Time | Onsite | Days | Monday, Wednesday, Friday 7:00 AM – 4:00 PM; Tuesday, Thursday 9:00 AM – 6:00 PM | 40 hours per week | Minimum 12 weeks, start September

Key Responsibilities

  • Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction.
  • Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines.
  • Design functional treatment programs focused on progressing patients toward discharge within an established length of stay and facilitating effective return to community activities.
  • Delegate and direct all discipline-specific therapy services for assigned patients.
  • Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations.
  • Adhere to discipline-specific State Practice Act and all regulatory agency requirements, and actively pursue professional development through continuing education and professional activities.

Required Qualifications

  • Active and unrestricted Physical Therapist licensure in good standing in the applicable state.
  • Minimum of 1 year of clinical experience as a Physical Therapist.
  • Ability to perform comprehensive patient evaluations and develop appropriate, goal-oriented treatment plans.
  • Proficiency in medical record documentation in compliance with state, federal, and payer guidelines.
  • Completion of all state-mandated continuing education requirements and adherence to the discipline-specific professional code of conduct.

Preferred Experience / Skills

  • Prior experience in an outpatient physical therapy setting.
  • Experience supervising or mentoring physical therapy students.
  • Familiarity with performance improvement methodologies and quality initiatives in a clinical environment.
