Key Responsibilities

• Conduct comprehensive patient evaluations and develop, implement, and modify individualized treatment plans in alignment with patient goals and physician direction, including 60-minute eval and treatment sessions
• Complete accurate and timely medical record documentation including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
• Delegate and direct discipline-specific therapy services for assigned patients, ensuring care is provided under valid physician orders and within insurance approvals
• Design functional treatment programs that support patients' effective return to community activities within an established length of stay
• Serve as an educator for patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations
• Adhere to discipline-specific State Practice Act and all regulatory requirements; maintain professional licensure in good standing and fulfill all state-mandated continuing education requirements

Required Qualifications

• Active Physical Therapist license in good standing in the state of Texas
• Demonstrated ability to perform comprehensive patient evaluations and develop evidence-based treatment plans
• Proficiency in clinical documentation including goal writing, functional assessments, and charge entry
• Commitment to patient and employee safety standards and safe patient handling practices
• Adherence to discipline-specific professional code of conduct and ethical standards

Preferred Experience / Skills

• Prior experience in an outpatient physical therapy setting
• Experience supervising or mentoring PT students in a clinical environment
• Familiarity with performance improvement methodologies and quality initiatives
