Job Summary
Physical Therapist Inpatient (PT IP) located in the Yolo County area. This role involves providing physical therapy services to patients in a home health setting, focusing on assessing, evaluating, and developing therapeutic plans of care according to physicians prescriptions. The position requires a committed professional to deliver quality care in a community-based environment.

Job Details

• Position Description: Provides physical therapy services according to a written physicians plan of care, including evaluating patient status, assessing home environments, instructing patients and families, and participating in case conferences. Supervises Home Health Aides and refers patients to other home health services as needed. Prepares and submits clinical and progress summaries, reports patient reactions to treatment, and collaborates with healthcare team members.
• Required Qualifications: Certification in BLS; 2 years of experience as a Physical Therapist in an acute, home care, or rehabilitation setting; valid license; location-specific requirements; minimum 1 year of related experience.
• Schedule: Days shift, 8 hours per day, with a minimum of 40 hours per week. Usually rotate PTs on Saturdays once a month.
• Additional Information: Candidates must have good oral and written communication skills and organizational abilities. Scrubs are not required; dress code is slacks and a polo or button-up shirt. Mileage will be reimbursed at the GSA rate with prior approval.
• Submission Requirements: Candidates need to submit at least two supervisor referencesone within 12 months and one within three years of applicationand must not have been directly employed by any facility associated with the organization within the past year.
