Job Details:

• Schedule: Days shift, 8 hours per day, minimum 40 hours per week.
• Required Qualifications: Degree in Physical Therapy from an Accredited School; 1+ Years of Outpatient Clinical Experience; CPR Certification; active/applicable license for the State of Connecticut.
• Preferred Qualifications: Successful completion of previous assignments with the agency; previous assignment with the organization.
• Additional Information: Performs comprehensive patient evaluations, collaborates on treatment plans, completes accurate medical record documentation, and ensures care is provided per physician orders and insurance approvals. Participates in student supervision and professional development activities, adhering to state practice acts and maintaining licensure in good standing. Demonstrates engagement with performance improvement initiatives and fosters a safety-first environment.
