We are seeking an experienced professional to provide exceptional care to our patients in the comfort of their homes. This part-time position offers the opportunity to make a meaningful impact in the community, servicing various locations in and around Plainsboro, NJ.

Responsibilities

• Conduct comprehensive assessments and develop individualized care plans for patients.
• Provide physical therapy treatments to patients in their homes, ensuring high-quality care and patient safety.
• Collaborate with other healthcare professionals to optimize patient outcomes.
• Educate patients and their families on therapy techniques and self-care practices.
• Maintain accurate and timely documentation of patient care activities.

Requirements

• Minimum of 2 years of experience in physical therapy, with start of care and revisit experience required.
• Active PA license required to submit.
• Basic Life Support (BLS) certification required.
• Advanced Cardiovascular Life Support (ACLS) and Neonatal Resuscitation Program (NRP) certifications required.
