Key Responsibilities

• Provide physical therapy services to patients in a home health setting
• Manage a caseload with an expected productivity of 28 points per week
• Oversee and supervise Physical Therapist Assistants (PTAs)
• Utilize HCHB (Homecare Homebase) electronic documentation system for patient records and care coordination
• Operate within a service area of up to 40 miles from the primary or satellite office location

Required Qualifications

• Active Physical Therapist license in the state of Connecticut
• Prior home health experience required
• Proficiency with HCHB (Homecare Homebase) documentation platform required
• Ability to work Monday through Friday, 8-hour day shifts with no on-call or weekend requirements
• Candidate must be located within a commutable distance to the Mansfield or Norwich area (service radius up to 40 miles)

Preferred Experience / Skills

• Experience supervising or collaborating with Physical Therapist Assistants
• Strong organizational and time management skills suited to an independent, field-based role
