Physical Therapist (Home Health)
Location: Dunedin, Florida
Contract Length: 13 Weeks
Schedule: Day Shift | 5×8 Hours (8:00 AM – 5:00 PM)
Hours: 40 Hours/Week
Seeking an experienced Physical Therapist (PT) to provide skilled home health services to adult and geriatric patients. The PT will assess patients, develop treatment plans, improve functional mobility, and help patients achieve maximum independence in their home environment.
Perform PT evaluations and reassessments
Develop individualized treatment plans and goals
Provide gait, balance, strength, and mobility training
Complete OASIS and home health documentation accurately
Assess home safety and identify environmental barriers
Monitor pain levels, vital signs, and patient progress
Educate patients and caregivers on exercises and safety measures
Collaborate with healthcare teams to ensure quality patient care
Manage approximately 6 patient visits per day
✔️ Active Physical Therapist License
✔️ Minimum 2 years of PT experience
✔️ BLS Certification
✔️ Strong documentation and time-management skills ⏰
✔️ Home Health experience preferred

Cardiopulmonary (CHF, COPD, CABG, MI)
Weekly Gross Pay: $2,636
W2 Hourly Rate: $26/hr
OT Rate: $39/hr
Weekly Per Diem: $1,596 (Non-Taxable)
High weekly gross package ($2,636/week)
Day shifts only
No travel experience required
Manageable caseload (6 visits/day)
