Key Responsibilities
- Provide physical therapy services within a CMS Certified Home Health Agency setting
- Conduct Start of Care (SOC) evaluations and complete OASIS assessments accurately and in a timely manner
- Develop and implement individualized patient care plans based on evaluation findings
- Deliver skilled physical therapy interventions to patients in a home health environment
- Document patient progress and clinical notes utilizing electronic health record systems
Required Qualifications
- Active Physical Therapist (PT) license in the state of Washington
- Prior Home Health experience required
- Demonstrated experience with SOC evaluations and OASIS documentation
- Proficiency with HCHB (Homecare Homebase) electronic medical records system
Preferred Experience / Skills
- Strong familiarity with CMS Certified Home Health Agency workflows and compliance requirements
- Experience working within a high-volume home health caseload
