Key Responsibilities

  • Provide physical therapy services within a CMS Certified Home Health Agency setting
  • Conduct Start of Care (SOC) evaluations and complete OASIS assessments accurately and in a timely manner
  • Develop and implement individualized patient care plans based on evaluation findings
  • Deliver skilled physical therapy interventions to patients in a home health environment
  • Document patient progress and clinical notes utilizing electronic health record systems

Required Qualifications

  • Active Physical Therapist (PT) license in the state of Washington
  • Prior Home Health experience required
  • Demonstrated experience with SOC evaluations and OASIS documentation
  • Proficiency with HCHB (Homecare Homebase) electronic medical records system

Preferred Experience / Skills

  • Strong familiarity with CMS Certified Home Health Agency workflows and compliance requirements
  • Experience working within a high-volume home health caseload
