Job Summary:
Physical Therapist Home Health (PT HH) in a community setting. This position involves providing therapeutic services to patients in their homes, focusing on improving mobility and relieving pain. The role requires delivering personalized care tailored to individual patient needs while working within established clinical protocols.

Job Details:

  • Schedule: Days shift, 8 hours per day, with a minimum of 40 hours per week
  • Required Qualifications: Valid California Physical Therapist license, BLS certification, and at least 0 years of experience in home health physical therapy
  • Certifications: BLS (American Heart Association or American Red Cross) must be current and uploaded with valid expiration dates
  • Additional Information: Caseload involves adult patients with diverse diagnoses requiring home-based physical therapy interventions. Clinical skills should include patient assessments, treatment planning, and documentation in electronic charting systems. Flexibility to work within scope and accommodate patient needs is preferred.
