Key Responsibilities

• Provide physical therapy evaluations and treatments to patients in a home health setting across an assigned geographic pod
• Actively manage a caseload to meet expected weekly productivity targets of at least 30 points per week
• Proactively backfill cancelled visits by contacting patients to reschedule or advance upcoming appointments
• Complete clinical documentation accurately and in a timely manner using HCHB (Homecare Homebase)
• Coordinate with the home office team to maintain a full and productive schedule throughout the week

Required Qualifications

• Active Physical Therapist (PT) license in the state of California
• Minimum 1–2 years of clinical experience as a Physical Therapist
• Proficiency with or ability to learn HCHB (Homecare Homebase) documentation platform
• Valid driver's license and reliable transportation for home visits

Preferred Experience / Skills

• Prior home health physical therapy experience strongly preferred
• Prior experience with HCHB documentation software
• Strong self-management skills and ability to independently manage a field-based caseload
