pay + IRS Mileage Rate $.76/mile

Schedule: Monday-Friday Guaranteed Work Week: 40
Coverage: Opal (Cottonwood south to Orland) May be asked to help in other territories within facility's service area as needed which is about a 60 mile radius from the Facility address. Average 4-6 visits per day depending on type of visit for PT/OT/SLP Productivity is 28-32 points per week for PT/OT/SLP SOC: 2.5 points, ROC: 2 points, Evaluations/Recert/Discharge OASIS: 1.5 points, Discipline DC, routine visit, 30-day reassessment: 1 point, Weekly case conference: 1 point Any overtime must be pre-approved.
Home Health and Oasis experience required, no exceptions Homecare Homebase documentation experience required Client may consider without HCHB exp at a reduced rate for the first 2 weeks if the manager is willing to train.
