Amergis Healthcare Staffing is currently seeking a Licensed Physical Therapist (PT) for an exciting 3-month contract opportunity supporting a PACE (Program of All-Inclusive Care for the Elderly) organization in Sacramento. This position offers the opportunity to make a meaningful impact by helping seniors maintain their independence, mobility, and quality of life while remaining in their homes and communities.

Position Overview

The Physical Therapist will provide evaluations, assessments, and treatment plans for participants aged 55 and older. Working as part of an interdisciplinary team, the PT will deliver restorative, maintenance, and rehabilitative services while integrating therapy goals into each participant's individualized Plan of Care.

Responsibilities

• Perform physical therapy evaluations and develop individualized treatment plans.
• Provide therapeutic exercises, functional mobility training, and other physical therapy interventions.
• Assess participants for acute, restorative, and maintenance rehabilitation needs.
• Conduct home visits and assess participants' safety, mobility, and equipment needs.
• Collaborate with physicians, nurses, occupational therapists, speech therapists, caregivers, and other interdisciplinary team members.
• Educate participants and caregivers on treatment plans, exercises, safety techniques, and durable medical equipment (DME).
• Monitor participant progress and modify treatment plans as needed.
• Participate in care plan meetings and interdisciplinary team discussions.
• Coordinate rehabilitation services with assisted living facilities, skilled nursing facilities, and community partners.
• Manage DME recommendations, tracking, and participant education.
• Maintain timely and accurate clinical documentation.
• Support quality improvement initiatives and participant-centered care planning.

Qualifications

✅ Active California Physical Therapist License
✅ Bachelor's Degree in Physical Therapy from an accredited program
✅ Minimum one year of experience working with geriatric, frail, or elderly populations
✅ Strong assessment, treatment planning, and care coordination skills
✅ Valid driver's license and reliable transportation for community and home visits

Preferred Qualifications

⭐ 2+ years of clinical Physical Therapy experience
⭐ MSPT or Doctor of Physical Therapy (DPT)
⭐ Experience in PACE, home health, geriatrics, skilled nursing, or rehabilitation settings
⭐ Bilingual skills are a plus
