Responsibilities
- Evaluate and treat patients with medically prescribed physical therapy programs to restore function and maximize performance within individual capabilities.
- Develop and document observations, clinical findings, problems, skilled interventions, goals, and discharge plans.
- May supervise Physical Therapy Assistants and conduct initial admission/evaluation visits.
- Notify the Clinical Team Manager of OASIS scores within established timelines.
- Collaborate with the care team to coordinate services and adjust plans based on patient progress and needs.
- Communicate clearly with patients, families, and team members to support continuity of care and discharge planning.
Requirements
- One year of Home Health experience
- Ability to perform initial admissions/evaluations and document findings
- Strong clinical documentation and communication skills
- Comfortable working in a home health and hospice setting
- Availability for a 13-week assignment with 40 hours per week
- Willingness to travel within the assigned territory as needed
