Description

American Traveler is seeking a Physical Therapist for a Home Health assignment requiring a CA PT license, EPIC and OASIS experience.

Details

Home Health setting covering the South Marin county territory, including Sausalito, Tiburon, Belvedere, and Mill Valley
13-week assignment with day shift schedule, 5x8-hour shifts from 8:00 AM to 5:00 PM
Caseload of approximately 5 visits/points per day
Uses EPIC EMR system with OASIS documentation required

Requirements

Active CA PT license required if applicable
Current BLS certification required
Valid driver's license and auto insurance required
Minimum 2 years of experience required
Prior travel experience required
EPIC EMR experience and OASIS documentation experience required
Experience with orthopedic, neurologic, and cardiopulmonary patient populations preferred
Skilled in home exercise programs, gait training, and patient/family education

Additional Information

Responsibilities include evaluation, treatment planning, and delivery of home-based physical therapy services to patients with orthopedic, neurologic, and cardiopulmonary conditions
Collaborates with interdisciplinary care team to develop and implement individualized treatment plans
Requires strong critical thinking, independent judgment, and time management skills to manage a home health caseload
Must complete required lab work within 3-5 days of contract signing
Maximum RTO of 7 days per contract, with only 1 holiday request approved per contract
Local candidates accepted at local rate; 50-mile radius rule applies for travel rate eligibility
Returning employees/travelers must have a minimum 1-year separation before being eligible to return
