Bay State Physical Therapy, 655 Main Street, Walpole, MA, 02081 Everything covering OP location. Birth to death Ortho with Geri's and athletic population also. Flexible Start Date Flexible Length of Contract Duration - 13-weeks to 26-weeks Guarantee- 40 hours/week Productivity - 55-63 patients/week, treatments are typically scheduled 2 per hour and evals are 1 hour. Schedule - Open to 5x8, 4x9+4, 4x10s; must be open to 2 evenings/week (until 7:00pm) Cancel Notice - 30 days EMR - Raintree Notes - Must have at least 3 months of OP exp or 1 year of PT exp in other settings post clinicals. All new travelers must complete three HealthStream Modules pre-assignment. Agency will be reimbursed with a $180 lump sum upon proof of completion and at least 1 shift worked.
