We are seeking a dedicated and compassionate Travel Physical Therapist Assistant (PTA) to join our home health therapy team. Working under the supervision of a licensed Physical Therapist, the PTA will provide individualized therapy services to patients in their homes, helping them improve mobility, strength, balance, endurance, and overall functional independence.
This is an excellent opportunity for a therapy professional who enjoys working independently and building meaningful relationships with patients while providing care in a home-based environment.
Responsibilities
Implement individualized treatment plans developed by the supervising Physical Therapist
Provide therapeutic exercises, gait training, balance activities, strengthening exercises, transfer training, and functional mobility interventions
Evaluate patient responses to treatment and communicate progress or concerns to the supervising Physical Therapist
Provide therapy services in patients' homes and adapt treatment approaches to each patient's environment and needs
Educate patients and caregivers on home exercise programs, mobility techniques, fall prevention, and safety strategies
Assist patients with improving functional mobility and independence with daily activities
Maintain accurate and timely clinical documentation in accordance with home health and regulatory requirements
Communicate effectively with Physical Therapists, nurses, physicians, caregivers, and other members of the healthcare team
Participate in care planning and discharge planning as appropriate
Manage a daily schedule of home visits while maintaining high standards of patient care
Maintain patient confidentiality and professional standards while working in the home setting
Requirements
Required: Graduate of an accredited Physical Therapist Assistant program
Required: Active PTA license in the state of assignment or ability to obtain
Previous home health/homecare experience preferred
Strong communication, documentation, organizational, and interpersonal skills
Ability to work independently and travel between patient homes
Reliable transportation and ability to meet scheduled home visits
Ability to adapt treatment to different home environments and patient needs
