Travel Physical Therapist (PT) - $1,925 to $2,125 per week in Poulsbo, WA

Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• WA license information and any other needed information for credentialing must be in place at a minimum.

Assignment Details

• 40 minute 1:1 patient care schedule.
• Medicare/Medicaid credentialing is preferred. If not already credentialed, WA license information and any other needed credentialing information must be in place.
• New graduates will be considered based on licensing and clinical experience.
