PT for Home Health Signature Healthcare at Home Home Office Address: 7632 SW Durham RD, Tigard OR 97224 The PT will be responsible for an everage of 2-5 patients per day, whether its an eval/start of case or subsequent visit. The weekly points expectation is 25 points per week. EMR is HCHB Pointcare. PT will also be responsible for supervision of 1-2 PTA. The area of coverage needed is East Portland with a 30-mile radius. Orientation wise, there will be 1-3 days orientation depending on need plus compliance training videos and ride alongs.
