We’re looking for a Physical Therapist to take on a full-time role with an area home health client in Redmond, WA. You will help patients move better, feel better, and stay active—whether they are recovering from an injury or managing long-term conditions. Minimum Requirements:

• Bachelor’s, Master’s or Doctoral Degree in Physical Therapy from an accredited program.
• Active WA State Physical Therapist license required to start the position.
• BLS certification from AHA or ARC may be required.
• Experience providing home health physical therapy or equivalent setting experience preferred.
• Ability to complete OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition.
• Experience appropriately utilizing ICD-10 coding.
• Experience supervising PTAs and Home Health Aides preferred.
• Comfort managing a caseload of approximately 25 visits per week (20 visits per week when paired with a PTA).

Responsibilities:

• Provide physician-ordered plan of care under the direction and supervision of the Director of Clinical Services.
• Work in conjunction with home care personnel in patient care planning.
• Direct and supervise Physical Therapy Assistant and Home Health Aide, as assigned.
• Take initial patient history and perform initial evaluations.
• Consult with physicians regarding changes in treatment.
• Instruct patients and families in home programs and activities of daily living.
• Participate in Quality Assessment and Performance Improvement activities as assigned.
• Complete and submit required OASIS documentation and appropriately apply ICD-10 codes.
