Minimum Requirements:
- Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited school approved by the APTA.
- Active Washington State Physical Therapist license required to start the position.
- BLS certification from AHA or ARC may be required.
- Experience in home health setting preferred.
- Ability to provide physician-ordered plan of care and complete OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant change in condition.
- Experience supervising PTAs and Home Health Aides and delegating tasks appropriately.
- Competence with ICD-10 coding.
- Willingness to travel and cover the Redmond area as part of the caseload.
- Valid driver’s license and reliable transportation for community/home visits.

Responsibilities:
- Provide physician orders and implement plan of care under the direction and supervision of the Director of Clinical Services.
- Work collaboratively with home care personnel in patient care planning.
- Direct and supervise Physical Therapy Assistant and Home Health Aide, as assigned.
- Take initial patient history and perform initial evaluations.
- Consult with physicians regarding changes in treatment.
- Instruct patients and families in home programs and activities of daily living.
- Participate in Quality Assessment and Performance Improvement activities as assigned.
- Complete and submit required OASIS documentation and utilize ICD-10 codes appropriately.
