This 13-week Physical Therapist opportunity is based in Greensboro, North Carolina, a city that blends Southern charm with urban convenience. The assignment is within a skilled nursing facility focused on rehabilitation and long-term care. You will work with a collaborative team of nurses, therapists, and other healthcare professionals who value evidence-based practice, clear communication, and patient-centered care. You will work a day schedule, 5 eight-hour shifts per week, with an expected 40-hour workweek and weekly billing/pay cycles. The caseload typically consists of adult and geriatric patients requiring a range of services including post-acute rehabilitation following orthopedic or medical events, chronic disease management, and functional mobility training to maximize independence and safety. While specific patient ratios may vary, the goal is to maintain a manageable caseload that allows for thorough evaluations, effective treatment sessions, and timely documentation. Daily responsibilities include performing comprehensive PT evaluations, developing individualized treatment plans, and delivering skilled physical therapy interventions based on patient goals and clinical presentation. You will provide therapeutic exercises, gait and balance training, transfer training, pain management strategies, and functional task practice to support safe mobility and self-care. You will regularly reassess progress, modify plans of care, and contribute to discharge planning, whether to home, assisted living, or higher levels of care. Collaboration is central to the role. You will participate in care conferences, coordinate with nursing, occupational therapy, speech therapy, and social services, and communicate clearly with families regarding functional status and safety recommendations. Accurate and timely documentation in the facility’s EMR system is expected, reflecting skilled interventions, progress toward goals, and compliance with regulatory requirements.
