Travel Physical Therapist (PT) - $2,079 per week in Medford, NY

The physical therapist will conduct home visits and attend periodic office meetings. The average number of patients per day is 5 to 6. Contractor will be assigned between 5 to 10 patients per day, equating to roughly 6 to 8 points. There are 30 points expected per week, with each type of visit and activity (documentation, travel, etc.) holding a different point value. Types of cases include Start of Care and Follow Ups. First-time applicants for this position are not accepted; recent home health experience is required. Weekend requirements vary by location and profession. Travel up to 75 miles from the home clinic is necessary, and mileage will be reimbursed at the IRS standard rate. Local candidates are accepted at the same rate. Candidates must be ready for on-call rotations, which vary based on the job or location. Documentation must be completed within 24 hours of appointments with patients. Delayed and incomplete documentation can impact billing and follow-up patient care. It is important that candidates are aware that there are no guaranteed hours with this contract. Weekly productivity points of 30 must be achieved, equating to 40 hours per week. If insufficient patients are assigned, there will be opportunities to float to another nearby clinic to pick up patients. Experience with HomeCare HomeBase EMR is preferred.
